Funeral Home Survey
Help families have information to better plan their babies' services

You may download this survey and use it to gather information from your local funeral homes.  This in turn can then be collated and given to hospitals to share with families shortly after their baby dies.  In this way, they can make an informed choice rather than just selecting a funeral home from the phone book.

  • Send or deliver a copy of the survey to each funeral home in your area and tell them the purpose is to help newly bereaved families in the hospital make informed choices when selecting a funeral home.
  • Explain that if they answer it and return it, their home will be represented in the book, otherwise not.
  • Put the copies in plastic sleeves, into a 3 ring binder and give it to your local hospital L & D, Post Partum, NICU to share with families at the time they begin planning their baby's service.  You could charge a small amount to cover the printing costs.

Funeral/Cemetery help

Tears - A new organization has started in Washington state.  Its main mission and focus is  to lift a financial burden from newly bereaved families. "We compassionately assist newly bereaved families whose baby has died by providing funds to assist them with the cost of burial or cremation expenses.   Our goal is to open up Chapters in other States and eventually meet a national need."

The Angel Names Association is a nonprofit organization dedicated to assisting families of stillborn children through programs designed to provide financial assistance for end-of-life expenses and counseling services, and funding for stillbirth research.  It is based in NY.  http://www.angelnames.org
